Why This Intake Matters

One intake. Seven readiness domains.

This intake form gives The DeAngelo Group a structured baseline before the assessment begins. It helps identify existing plans, known gaps, operational concerns, training needs, communication issues, threat and hazard priorities, and items that may require immediate follow-up.

The information provided helps prevent guessing, keeps the assessment focused, and allows the review to be tailored to the organization, site, people, operations, and risk environment. This is only the starting point. Additional questions will be handled during the in-person assessment, site walk, interviews, or follow-up meeting.

The questions are organized under the P.R.O.T.E.C.T. model so each response connects to a specific readiness domain. Primary questions are numbered by section, and when a response is marked Yes, one follow-up question appears for baseline detail. The client is not being asked to select a service type at this stage. The intake is intentionally broad so The DeAngelo Group can determine the right path during review or during the in-person assessment.
